I believe the solution lies in both setting the ground rules and then natural consequences.
I teach Entrepreneurship, Economics, Marketing, and Intro to Canadian Business in a formal classroom setting.
I start each course of the same with the "your an adult and I expect you to be a good neighbour" speech. I do provide the subtext that these are the rules for my class, and are not transferable to other classes or instructors.
In regard to Loud talkers/disruptive students the preparation speech lets them know that there are often times when you need to ask a neighbour a question, make a comment, etc. Do so discretely and briefly. I usually talk fairly loud so there are not many times when the individuals that talk will be louder than me. If the conversation goes on too much or too loudly I allow students to do the stare down first. Often times this makes the talking students uncomfortable and they stop. The next step in the natural consequences, is usually for a student or students from the class to "shhhh" the talking students. The last progression involves me saying "sshhhh" or creating an uncomfortable pause in my lecture that draws attention to the students that are talking and they usually stop. If necessary after this part, I will say something directly to the student, like "time-out" your neighbours are trying to hear this part. To date I have not had any issues with maintaining a respectful classroom. Maybe I am threatening looking enough they dont test me but I prefer to think that I am entertaining enought to keep them engaged.

I agree in many ways that a lot of the learner centred approaches are excellent in the right circumstances. However, like Tom said, there are constraints that minimize or completely eliminate their use.
